General Information
All athletes and volunteers that wish to participate in any our programs are required to fill out the
appropriate enrollment forms. Completed forms may be mailed (please see the forms for mailing
instructions) or may be submitted to one of our organizers at the program sessions, prior to
commencing the athlete/volunteer's participation.
Athlete Enrollment
All athletes are required to fill out the Athlete Registration form available by
clicking Athlete Registration Form. This is a common form for all our sports
activities.
Please print this form, fill out all the sections and either bring it to any of our
sports sessions or mail it to us (Please see Contact Us tab for mailing info)
Volunteer Enrollment
All volunteers are required to fill out the Employee/Volunteer Disclosure
Statement form available by clicking Volunteer Form. This is a common form for
all our sports activities.
